How to Apply
- The Mentor applications are avaiable below. Download the appropriate application from the Mentor Web page and submit it to jbrown@practicegreenhealth.org.
- Provide qualitative and quantitative data to demonstrate success in your preferred area of focus (as described in application).
- Notification of selected Mentors will be made within four weeks of application submission.

- Is the Mentor Program for Business Members as well?
While the program is designed to help hospitals meet their goals, Practice Greenhealth business members ARE eligible for mentoring. The program is open to all members of Practice Greenhealth – businesses and health care providers alike!
- Does it cost anything to participate?
No, this is a Practice Greenhealth membership benefit.
- Who will run the program?
The Program will be run by Practice Greenhealth staff, including coordination of calls, note-taking and follow up.
- Do you have to be a member to participate?
Yes, the Mentor Program is a benefit for Practice Greenhealth member facilities only. A similar program is under development for the challenges of the Healthier Hospital Initiative. Go to the Healthier Hospital Initiative Website for more information.
